Upload image in list view with image picker

i try to make a medicall report using list view with an image and the details, how do i upload the image in list view with image picker? i only can upload it manually through designer section

i wanna make it the list like this

but i insert the pic and the text manually in designer section, i want to insert the text and image with image picker so i can take the pic from the gallery's phone

Be aware that the image picker only stores the last 10 images. If you need more than this, then consider this workaround:
