Two Label as conditions to notify

Hello gentlemen,
I am working on my project about notifying just at a specific items with two conditions. I just knew to add only one condition and I need to add one more condition for notifying.
I put two labels as an example of two conditions. Label1: 49 and labe2: 94.
and I need to put three column in csv. the first column for label1 and column2 for label2 and the third one is to show what is notifying.

The idea is When for example : Label1: 49 and Label2:94 then notify head2.

I Tried to build double blocks for two conditions but it seems that would be duplicated.
