The MIT app inventor says unable to load image and sound file, using the files from the pltw and off google, it works on 3 of the 8 ipads, all are on version 2.68, and have similar settings, all while using the MIT AI2 Companion to try and play my code. I did clear the cache and it seemed to have no effect.

For iOS, the latest version is 2.78.3. Version 2.68 is fairly old so you would likely benefit from upgrading to the latest version to get a myriad of new features and bug fixes.
