Appstore people ask a video of the following :
- The current version of the app in use on a physical Apple device, not on a simulator
- The initial pairing process between the app and the designated hardware
- The entire app workflow with the designated hardware
but i can’t find a way to download my app to test it on an iphone and then film the required elements.

Build the IPA (iOS Ad Hoc) with an Ad-hoc provisioning profile and install it.
You really should have done that already — after all, how else did you get your screenshots for the App Store?
Can you ask the Appstore people how to download your app in order to make the video for them?

Ok, so where is the problem to install the IPA on your iPhone?
You can also use TestFlight to install your app as a beta test on a device and record the video that way before pushing it to App Store review.

Okay i did all of that, but now i need to rebuild the app since apple only support BLE to connect with an esp 32
That sounds like a strange requirement to me. What was the message they gave you?
