Base URL to form looks OK. Opens up the form here. Can you open the form in your browser (incognito/private window, or not signed in to Google account) ?
You did have this working in your other topic, so what has changed since then?:
( I notice you do not appear to be using the replacement blocks to fix your decimals....although that wasn't causing a problem with the URL)
