Bad arguments to list to csv table
The operation list to csv table cannot accept the arguments: ['Carimbo de hora/hora, Rating']

The error comes because you use list to csv table list instead of list from csv table list block

It looks like it solved half of the problem.
Because the data is not saving in the Google sheet...
We can't see your full url, but if you only have 1 element on the google form you should use a ? and not an &.
