Can you please post high-resolution blocks? Or atleast a clear portion of the blocks involved.

Start simple, get it running, then add more logic
For example have 3 phone numbers in a list
Create a background process using the Clock.Timer event, use a timer interval of 10 seconds and send a sms to the next number on each timer event

the issue is that while the app is foreground it sends. Its when its in the background it doesnt. It waits till its foreground again.

and get it running in the background

Remember, there does not exist any user interface in the background... no labels, no textboxes etc

Now combine those 2 examples to send sms in the background

Here is a working example for you
each 10 seconds the test app sends a sms in the background to another phone number until the phone number list is empty
Adding Notifier.LogInfo blocks help to debug the app using logcat, see also Capturing Device Logs using adb logcat

Probably because up to now you did not really understand how background processing works?
Can you see procedure Clock1_Timer? This is an event handler for the Timer event of the clock component which will be called from itoo in the background

Just add a few test phone numbers, build the project, install it and start testing.
What about providing some feedback?
And after seeing it working, then add more logic step by step
