Seemed we are at crossed purposes.
We wanted to see the response after you submit the form, not the data in your spreadsheet.
If you have things setup correctly, your spreadsheet should have a sheet named Form responses 1, the data sent to the form should be stored there for example:

You can always use the actual google form to send some test data to your spreadsheet. If that works, then you know the issue is elsewhere.
As I suggested, try with the actual google form.
