It is working, but you, again, have some empty form submissions (delete rows 2,3,4 and go into the google form and delete the empty responses) and for some reason you have submitted the form with the column headers?
done that, but hows it working when it keeps saying name, songname, artistname when the forms submitted?
I just used the actual form to submit a request, which came through, but you still have many blank rows above it
but how would it work within the app without it saying name, songname, artistname
instead it would show up with something like you submitted
Seems there is an issue with your blocks and your editing of the pre-filled url from the google form. Please show all your relevant blocks, and the pre-filled form link in full.

If this works for you, (which it does for me) then apply the same logic and values to your app's blocks.
Do not touch anything on either of your worksheets: requests or req.
You can do that, or just compare, check, edit your current blocks to match.
