App not working

When I take a photo in the app, the image does not load. Also, when I click the 'Create PDF' button, an error occurs. I have added 5 buttons to capture images, but after taking a photo, it does not load. When I press 'Create PDF', an error appears. When I click 'View PDF', another error occurs. The image is not being set to the place where the label is. I have attached the AIA file of the app along with this. If possible, please help me fix this so that it works properly.pls share fix aia .
